Field Experience Evaluation Forms

This form combines both the student's evaluation as well as the supervisor's evaluation of the field experience. This evaluation must be completed, signed, and submitted for each semester of field experience, practicum, or internship.

Field Experience Agreements

The SDSU School Psychology Program uses the following field experience agreements to establish formal relationships with field supervisors and sites in order to ensure that expectations between the university, the field supervisor, the supervisee, and the school site are clear.  Each agreement delineates the responsibilities of each concerned party and specifies the nature of the experiences that are both appropriate for and expected from each student trainee at each level. 

There are three field experience agreements on this page covering three distinct phases including: Second Year Field Experience for CSP 730, Third Year Practicum for CSP 752, and Fourth Year Internship for CSP 780.

Evaluation of Supervisor Form

This form is used by program trainees to rate their experiences with their field supervisors and their site.  This form can be downloaded as a text file or may be picked up in the department office.

To download and use this form as text files, you must have either MS Word v. 7.0 (came with MS Office '95) or higher if you are using an IBM/PC compatible or MS Word v. 6.0 (came with MS Office '96 for Mac) if you are using a Mac.  You should be able to open the file as well if you are using another word processing program and have the very latest version (a version that came out after the versions listed here).  The BEST way to open these files is to save them to your disk rather than having them open up directly with Word.  Your browser may ask you about this.  After a file has downloaded, start your word processing program, then choose the "File" and "Open" commands from the top menu.  Navigate your directories/folders to go and find the file you just downloaded (wherever you saved it to).  Note: If you are using a PC you may need to tell your dialog window to look for "All files" rather than to look just for native format files.  Select the file when you find it and it will open up in your program and will be converted automatically.

Note: You may have to do some minor editing to the form to restore original formatting, for example, inserting/removing tab spaces, changing column widths slightly, etc.  Much of this will depend upon what printer you will be using to print the form.  After you have edited the form to your needs, save the file normally and it will be in native format from then on.
